Tratamiento de la úlcera del pie diabético utilizando Heberprot-P

Para la úlcera del pie diabético, Heberprot-P es un fármaco desarrollado por el “Centro de Ingeniería Genética y Biotecnología (CIGB) en 2007 en Cuba.

La diabetes ya no se considera mortal; no obstante, la diabetes y las complicaciones derivadas de ella, cuando no están controladas, siguen siendo la sexta causa principal de muerte, con una tasa de mortalidad superior a la de algunos cánceres como el cáncer de próstata, cáncer de mama y el cáncer de colon. Una de las complicaciones más comunes y graves de la diabetes es la úlcera del pie diabético, que afecta aproximadamente al 15% – 20% de las personas con diabetes y con frecuencia conduce a la amputación del miembro inferior (alrededor del 84% de las amputaciones de las extremidades inferiores se deben a esta condición).

La úlcera del pie diabético ocurre cuando una úlcera o una herida se desarrolla en una zona de las piernas inferiores o los pies que no cicatriza o es difícil de tratar, lo que conduce a la exposición del tejido subyacente y lo vuelve susceptible a infecciones..

Síntomas y clasificación

No es inusual que las personas con diabetes tengan úlceras del pie diabético durante algún tiempo sin ser conscientes de su aparición. Algunos de los síntomas a los que debe prestar atención incluyen:

  • Uñas de los pies gruesas y amarillas
  • Mal olor del pie
  • Piel tensa, hinchazón, enrojecimiento u otros signos de infección
  • Dificultad para caminar
  • Dolor muscular o cramps que se producen con la actividad y desaparecen con el reposo
  • decoloración del pie
  • Úlceras o llagas en el pie

Según el Sistema de clasificación de Wagner, las úlceras del pie diabético se clasifican entre 0 y 5:

  • 0 – el pie es normal pero está en riesgo de desarrollar úlceras del pie diabético
  • 1 – ligera ulceración sin infección
  • 2 – úlceras con tendones y articulaciones expuestos
  • 3 – ulceración extensa con infección
  • 4 – gangrena de dedo(s) del pie o del antepié
  • 5 – gangrena del retropié o del mediopié

CAUSAS DE LA ÚLCERA DEL PIE DIABÉTICO

Uno de los principales factores contribuyentes para desarrollar la úlcera del pie diabético es neuropatía periférica.(daño nervioso) que surge debido a un aumento, aunque sea leve, del nivel de glucemia. Debido al daño nervioso y a la pérdida de sensaciones y de la capacidad de sentir dolor, los daños en el pie, incluso por causas tan simples como la presión del calzado o un pequeño corte, pasan desapercibidos y sin protección, lo que provoca empeoramiento de la pérdida de piel, ampollas y la posibilidad de una infección grave que puede progresar en cuestión de días.

Estrechamiento de las arterias (enfermedad arterial periférica) debido a la diabetes reduce el suministro de sangre a los pies y es otra complicación diabética que contribuye al desarrollo de las úlceras del pie diabético. Una lesión en una zona con mala circulación sanguínea tarda más en cicatrizar y es más difícil de tratar, lo que la hace propensa a desarrollar úlceras o gangrena (descomposición de las células debido a la falta de flujo sanguíneo).

FACTOR DE CRECIMIENTO EPIDÉRMICO (EGF) Y DIABETES

Numerosos estudios han demostrado que los pacientes diabéticos presentan una disminución en las concentraciones de factores de crecimiento, principalmente factor de crecimiento epidérmico (EGF). Naturalmente presente en el organismo, el EGF estimula y promueve el crecimiento y la regeneración celular al unirse a sus receptores: los receptores del factor de crecimiento epidérmico (EGFR). El factor de crecimiento epidérmico actúa como agente de cicatrización al acelerar la recuperación de las heridas al desencadenar la síntesis de colágeno, elastina y ácido hialurónico.

De ello se deduce que las personas diabéticas con una cantidad reducida de EGF tienen una mayor probabilidad de presentar cicatrización deficiente de las heridas, hasta extremos que pueden desarrollar complicaciones graves que requieran amputación del miembro inferior.

HEBERPROT-P

Heberprot-P es un producto cubano innovador con EGF como ingrediente principal, administrado a pacientes que padecen úlceras que no cicatrizan y no responden a otros tratamientos convencionales. Sigue siendo la única alternativa a la amputación para las úlceras del pie diabético. Alrededor de 160,000 pacientes con úlcera del pie diabético han sido tratados con Heberprot-P con éxito, reduciendo el riesgo de amputación en un 77%.

El uso de Heberprot-P para las úlceras del pie diabético tiene una influencia significativa en la calidad de vida global del paciente, ya que no solo acelera el proceso de cicatrización, reduce el número de cirugías necesarias para la eliminación de tejido dañado y disminuye el riesgo de desarrollar complicaciones como infecciones y gangrena, sino que también reduce el riesgo de amputación y los costos de hospitalización.

 

Heberprot-P se administra por vía parenteral y está libre de conservantes. Ha demostrado una relación beneficio–riesgo favorable y se ha demostrado que se tolera bien con efectos secundarios leves a moderados, siendo el dolor y la sensación de ardor en el sitio de administración los más frecuentes.

Según la Organización Mundial de la Salud (OMS), Heberprot-P fue registrado en Cuba en 2006 y ahora está registrado en 15 otros países, lo que ha permitido tratar a más de 100,000 pacientes; las solicitudes para su registro están en revisión en otros países, incluidos Brasil, China, la Federación de Rusia, Sudáfrica y los estados árabes del Golfo Pérsico.

Heberprot-P ha ganado la Medalla de Oro de la Organización Mundial de la Propiedad Intelectual (OMPI) y el Premio al Mejor Joven Inventor en la Exposición Internacional de Invenciones en Ginebra, Suiza.
